When placing orders please fill out the form below with as much information as possible. If there are any special instructions such as specific colors, brand, etc., please give me those instructions on the “Notes to Buyers”. Send it to your supervisor for approval (there is a place for Supervisor’s name and approval) and then email it to the buyer, CC-ing your supervisor as well.
